Vous avez dit cœliaque ? Avec ou sans O : 

Maladie Cœliaque en français ou Celiac disease en anglais

En ce mois de Mai, mois de la sensibilisation à la maladie cœliaque j’aimerai partager ce post pour contribuer à l’information du public et de nos pairs sur le sujet.

Patients :  pensez à signaler à votre médecin si vous avez les symptômes suivants (tableau 1).

Médecins: Pensez au dosage des anticorps anti-transglutaminase (avec dosage  IgA sérique) devant les symptômes évocateurs ou en cas de facteurs de risque (familaux ou maladies associées).

Dans notre cohorte du CHU Sainte-Justine ces dernières années, on note une augmentation du nombre de cas de nouveaux diagnostics d’environ 20% entre 2015 et 2018.

In adult patients with recurrent  Clostridium difficile infection, Fecal microbiota transplantation (FMT) performed better than Vancomycin alone or Fidaxomicin alone.

in this  open-labeled randomized trial published in the journal Gastroenterology, Hvas et al randomized 64 adults in one of the following three groups:

1- FMT by colonoscopy or nasojejunal tube, after 4–10 days of vancomycin (125 mg 4 times daily),

2- Fidaxomicin 200 mg twice daily  for 10 days

3- Vancomycin 125 mg 4 times daily for 10 days.

The primary outcome was combined clinical resolution and a negative result from a polymerase chain reaction test for Clostridium difficile toxin 8 weeks after the treatment.

The primary outcome was met in 71% of  FMT group,  33% in the Vancomycin group and  19% in Fidaxomicin group.

 

The future is bright for fecal transplantation in Clostridium difficile infection
